Admission Criteria
Qualifying age
55+
Qualifying Disabilities
Primarily Brain Injury (TBI or ABI), mental health diagnosis (Schizophrenia, Bipolar/Anxiety Disorders), or developmental disability (Down Syndrome, ASD)
Open according to program appropriateness
Qualifying Funding Sources
CADI Waiver, DD Waiver, private pay
Is physically, behaviorally and medically safe to participate in programming
Physically
Is mobile with or without a device
Behaviorally
Does not cause harm to self or others
Has no unmanageable wandering
Is able to be redirected
Is not actively abusing drugs or alcohol
Has set goals and is motivated to achieve those goals
Medically
Does not have a communicable disease
Is continent or has manageable incontinence
Has managed seizure activity
What is Day Program?
The SMB Day Program is a recreation, leisure and social program for adults with disabilities designed to provide quality care and life enrichment opportunities. Participants enjoy a variety of educational programming, group activities, crafts, exercise, community outings, and even time to relax if necessary. The Day Program is located on East Frontage Road next to Hwy 52 between 19th and 37th streets. Transportation to and from the program can be arranged through a variety of options.
Who do we serve?
The Adult Day Program is available to adults ages 55 and older who have been diagnosed with a neurological, physical or developmental disability, brain injury, or mental illness.
An initial interview and assessment will help determine their appropriateness for the program.
How Do I Get Started?
Step 1: Review program handbook.
Step 2: Request a program tour and meet and greet. Fill in the information requested in the email form by clicking the button below.
Step 3: Receive confirmation of placement on wait list.
Step 4: Once you’ve reached the top of the wait list, Day Program staff will contact you to set up an intake meeting.
