Day Support Services
What is Day Support Services?
SMB’s Day Support Services (DSS) consists of two programs, the SMB Urban Day Program and the SMB Rural Day Program.
The Urban Day Program is located on East Frontage Road next to Highway 52 between 19th and 37th Streets and is a social, recreational, and leisure program for adults with disabilities. SMB’s Day Programs are designed to provide quality care and life enrichment opportunities. Participants enjoy a variety of educational programming, group activities, crafts, exercise, community outings, and even time to relax.
The Rural Day Program is located on 75th Street NW at SMB’s main office and is a therapeutic activity program centered around a 2.5-acre micro farm and nature area. Participants can actively engage with each other and staff while learning how to grow and maintain various vegetable crops, herbs, and medicinal plants indoors and outdoors and determine how to develop what they grow into products such as lip balms, soaps, candles, salves, etc. Additionally, participants can care for and learn about various animals, both native and beyond. The Rural Day Program frequently takes trips to other farms or agricultural and nature sites, the Farmer’s Market, and local fairs and participates in other non-agricultural community engagement opportunities and activities.
Who do we serve?
SMB’s Day Support Services are available to adults ages 18-62 who have been diagnosed with a neurological, physical or developmental disability, brain injury, or mental illness. An initial tour, interview, and assessment will help determine if SMB’s DSS services are right for an individual and which program appeals most to them.
How Do I Get Started?
Step 1: Reach out to the DSS Program Director devin@smbrochester.org and request to review the program handbook.
Step 2: If the handbook is appealing, reach out to the DSS Program Director to set up a tour.
Step 3: After the tour, complete program application and receive confirmation of placement on waitlist, if there is one.
Step 4: Once you have reached the top of the wait list, Day Support Services staff will contact you to set up an intake meeting.

Admission Criteria
Qualifying age
18-62
Qualifying Disabilities
Primarily Brain Injury (TBI or ABI), mental health diagnosis (i.e., Schizophrenia, Bipolar/Anxiety Disorders, etc.), or developmental disability (Down Syndrome, ASD, etc.)
Qualifying Funding Sources
CADI Waiver, DD Waiver, BI waiver, private pay
Qualifying Support Needs
Must be physically, behaviorally and medically safe to participate in programming
Physically
Is mobile with or without an assistive device
Is able to transfer independently or with one staff assistance
Behaviorally
Does not cause harm to self or others
Does not display unmanageable wandering
Is able to be redirected from challenging behavior
Is not actively abusing drugs or alcohol
Has set goals, or wants to set goals, and is motivated to achieve those goals
Medically
Is continent or has manageable incontinence
Has managed seizure activity
